Abstract


MedConcert™ is the healthcare industry’s first multi-tenant, cloud based platform designed to connect and align all stakeholders in a meaningful way to scale and spread continuous performance improvement, streamline access to incentives, enable lifelong learning and care coordination across organizations and walls.

MedConcert bridges the Professionalism of Medicine with the Practice of Medicine, bringing the best practices from the world’s leaders in quality improvement, assessment and learning across critical areas of quality, safety and compliance including Maintenance of Certification and Quality Reporting.

CECity’s MedConcert is a platform that supports a holistic approach towards Continuous Performance Improvement across the healthcare continuum, including individual providers, cross functional teams, and disparate organizations. Through the alignment of Performance, Learning, and Incentives, MedConcert is being used to deliver programs in support of various healthcare models with a focus on not just measurement but Improvement. Programs being deployed through MedConcert™ are supporting shared risk models, such as Accountable Care Organizations, and other initiatives such as Pay For Performance Programs, Quality Designation Programs and Professionalism Programs.

In the proposed presentation, you will see how CECity and The American College of Physicians are using MedConcert to support a diabetes pilot test in ACP state chapters. Working closely with internists and their practice teams, the pilot engages the full power of MedConcert to improve diabetic patient care processes and outcomes. Applications (Apps) will include ACP’s innovative Medical Home Builder along with an automatically populated PQRS-based diabetes registry, patient survey tools, and a Facebook-like communication networking capability. In addition, pilot participants have access to pay-for-performance and recertification opportunities.

This presentation will explore how MedConcert provides healthcare providers and organizations access to a cost-effective, scalable platform in which Apps can be plugged in to address a wide variety of critical quality, safety and financial needs. Apps include patient surveys, clinical registries, quality reporting, population health management and coordination of care applications, to address issues such as reducing unnecessary hospital readmissions through enhanced care coordination.
