Learning Critical Thinking with Interactive Medical Cases

Abstract
Learning is enhanced and retention improves when the educational material is relevant, engaging and includes interaction. Building on the increasing capacity of the online environment, we have developed a series of 20 interactive medical cases. These interactive cases present clinical cases that are diagnostic puzzles, designed to let the reader determine the diagnostic and treatment plans. The format recapitulates a clinical encounter by presenting the patient's history with evolving results of the physical examination and laboratory and radiographic tests. Each case presents clinical problems that reinforce the process of thoughtful clinical decision making and illustrates the links between pathophysiology and both diagnostic testing and therapeutics. Multiple-choice questions require reflective analysis of the data, and address both differential diagnosis and management. Immediate feedback is provided to indicate why the selected response is optimal or is likely to be unhelpful. Scoring allows users to gauge their relative performance in comparison to their peers. Videos and interactive graphics are used to help the user understand the mechanistic underpinnings of the disease process and to reinforce critical thinking. We will illustrate some of the cases and share survey and usage data from more than 250,000 utilizations that reflect the reaction of the international medical community to this unique learning tool. http://www.nejm.org/multimedia/interactive-medical-case
